Why have you written to request my medical details?

If we have written to you with a lifestyle and medical questionnaire it is because the trustees of your scheme have given us permission to collect this information from you.

You’ll know this questionnaire has come from us as your trustee will have written to you or your partner first. When we send our questionnaire, it will come with a covering letter and a 'frequently asked questions' leaflet. They will all be clearly branded with our logo.

We have designed the questionnaire to be simple and quick to complete. Please complete and return it in promptly in the reply paid envelope provided. The information you supply will be used to calculate details of the policy the trustees have purchased or are considering purchasing from us. It is not used to calculate the benefits you will be paid, such as the pension payable to you, the pension that would be payable to any eligible dependants upon your death, and any future increases to these pensions. These have already been stated by the pension scheme rules and will not change, whatever medical or lifestyle information you share.

Once you have returned your questionnaire to us, in a small number of cases, our in house underwriting team will call you to get some more information. This call will normally only take about 15 minutes.

Any information you supply will be treated in confidence and will not be shared with the trustees of your scheme.
